It is with pleasure that we are welcoming Dr. Bilcea to Meadowlands Family Health Centre.
We will give priority to former patients of Dr. Eom to sign up with Dr. Bilcea.
In order for the former patients of Dr. Eom to sign up with Dr. Bilcea, you need to complete an enrollment form.
To get an enrollment form, you can do one of the following:
1. please email mfhcdocuments@gmail.com to request a form (PREFERRED). We will try to get the form emailed back to you within 2 business days. You can also email the completed form back to us at the same email address.
2. If you do not have access to internet, please come by the office for a blank form to be filled out.
3. Book an appointment with Dr Bilcea. Arrive early to complete the form prior to the appointment time. You are encouraged to book a meet appointment with Dr Bilcea through the ONLINE Booking option on our website. Please do not call us to book an appointment but use this online option, if possible, as we expect a high volume of calls.
You must return the Enrollment form by JUNE 30, 2023 to ensure that you have a spot in Dr. Bilcea’s practice. After this date, we cannot guarantee that we will be able to accept you back into the clinic.
